GROUP LIFE CIAIMS REQUIREMENTS
First thing’s first. Make sure you have all the right details and documents ready, so you can easily report the incident to us.
Depending on what you are submitting a group life and personal accident claim for, there are different requirements you will need.
I. In Case of Death Claims:
- Claim form
- Discharge slip
- Accident report, if applicable
- Police report, if applicable
- Original death certificate or UAE-attested overseas report
- Post-mortem report
- Medical report to confirm the diagnosis and medical history
- Passport copy
- Visa copy
- Emirates ID copy
- Repatriation bills copy

If death has not occurred in UAE, the following additional documents would be required:
The original Death Certificate must be submitted duly notarized and attested by the UAE Embassy in the country where death took place. Subsequently, the same should be attested by the Embassy of the country in the UAE and Ministry of Foreign Affairs in UAE
II. In Case of Temporary Total Disability Claims:
- Claim form
- Accident report
- Sick leave certificates
- Police report, if applicable
- Passport copy
- Visa copy
- Emirates ID copy
- Copy of the last three salary slips

III. In Case of Permanent Total and/or Partial Disability Claims
- Claim form
- Accident report, if applicable
- Police report, if applicable
- Medical report to confirm the degree of permanent disability
- Medical report to confirm the diagnosis and medical history
- Medical bills and receipts
- Doctor prescription
- Passport copy
- Visa copy
- Emirates ID copy
- Copy of the last three salary slips

- In Case of Critical Illness or Terminal Illness:
- Claim form
- Medical report to confirm the disease
- Medical report to confirm the medical history
- Passport copy
- Visa copy
- Emirates ID copy
- Disclaimer: Critical illness rider can be claimed only once the survival period is completed.
